Job SummaryWe're seeking a highly skilled Primary Care Physician to join our team. As a key member of our organization, you will be responsible for providing exceptional patient care, leading our care teams, and contributing to our mission of delivering better health to our patients.
Key Responsibilities- Provide high-quality, patient-centered care to a panel of up to 450 patients, focusing on acute and chronic illnesses in older adults.
- Take full accountability for patient outcomes, seeking consultation from specialists when needed, and staying involved in patient care.
- Engage with hospitalists, care teams, and other healthcare professionals to ensure seamless care coordination and patient outcomes.
- Lead your care team, including care promoters, facilitators, and coordinators, to deliver exceptional patient care.
- Participate in quality improvement initiatives, risk management programs, and clinical meetings to promote patient health and company goals.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Current, active State Medical License for the state(s) in which you will be working.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty (Board Eligibility required).
- Current DEA number for schedule II-V controlled substances.
- Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC) within the first 90 days of employment.
