This is a challenging and rewarding role that requires strong operational and case management skills. As a Case Management Liaison, you will be responsible for acting as an operational liaison between select out-of-scope SSM Health teams and providing coordination across the care continuum with a focus on case management.
Key Responsibilities- Manage internal and external stakeholder relationships to communicate progress, risks, issues, and mitigation to Care Transformation (CT) leadership.
- Escalate issues, risks, disputes to the CT leadership as appropriate.
- Maintain knowledge of latest best practices and industry insights related to case management for purposes of continued strategic planning and innovation.
- Support CT leadership in strategic planning, innovation, goal setting, and other areas.
- Maintain an ongoing awareness of select regulatory requirements to ensure compliance.
- Recruit, motivate, develop, and retain a superior functional leadership team.
- Perform other duties as assigned.
- Bachelor's Degree.
- Seven years' experience with five in leadership.
- Registered Nurse (RN) issued by Compact State or Registered Nurse (RN) - Oklahoma Board of Nursing (OBN).
