Position Summary
Responsible for ensuring the safe and effective administration of medication therapies through collaboration with the healthcare team. Evaluates physician orders for pharmaceutical treatments and oversees these therapies for drug selection, dosage, interactions, adverse reactions, and adherence to formulary guidelines. Acts as a drug information resource for nursing staff, physicians, and other healthcare professionals.Key Responsibilities
- Conducts thorough reviews of relevant biomedical literature, formulates appropriate responses, and provides timely and concise answers to drug information inquiries.
- Employs effective patient education strategies to counsel patients and their caregivers, covering medication therapy, potential side effects, adherence, proper usage, handling, and administration of medications.
- Ensures accurate communication between shifts regarding unresolved issues that require follow-up actions.
- Gathers necessary information to create medication therapy plans and identify any medication-related issues with existing treatments.
- Designs, recommends, monitors, and assesses patient-specific pharmacotherapeutic plans. Implements recommendations effectively, adhering to existing clinical intervention policies and procedures, and documents appropriately in medical records.
- Participates in the management of acute medical situations.
- Mentors pharmacy students and residents.
- Prepares and dispenses medications in compliance with best practices, organizational policies, and state and federal regulations. Manages medication therapies according to clinical guidelines.
- Verifies the accuracy of the work performed by pharmacy support staff in accordance with organizational policies and procedures.
Physical Requirements
Primarily operates within the pharmacy environment. Responsible for delivering medications to nursing units, which involves considerable walking, standing, and pushing a mobile cart weighing up to 50 pounds, as well as lifting materials weighing up to 25 pounds. May require frequent stretching, bending, squatting, or kneeling to perform job duties. Utilizes appropriate personal protective equipment (PPE) to minimize exposure to hazardous substances and takes necessary precautions to avoid needle sticks, minor cuts, or bruises. May encounter various conditions throughout the workday, including walking on hard surfaces, climbing stairs, and exposure to varying temperatures and humidity levels. Must possess the ability to perform manipulative skills such as writing, typing, and data entry into computer systems and automated dispensing machines. Requires the use of both hands, clear vision to read small print on medication packages and syringes, and effective verbal communication skills. Must be able to hear normal sounds amidst moderate background noise. This position involves rotating shifts.
Education, Experience, and Certifications
A Bachelor's Degree in Pharmacy or a Pharm D. from an accredited pharmacy school is required. Must be licensed in the state of North Carolina or eligible for reciprocity. Previous experience in a hospital pharmacy setting is preferred. Strong verbal communication and interpersonal skills are essential for establishing effective working relationships with patients, visitors, and other healthcare team members to ensure optimal patient care. Medical Center Rx only Immunization Certification is required.
