This position is a full-time opportunity to work as a Registered Nurse in a skilled home care setting. The successful candidate will be responsible for evaluating patients for appropriateness of skilled home care services and implementing a comprehensive home care plan based on the patient's needs.
Key Responsibilities:- Respond to referrals in a timely manner and evaluate patients for appropriateness of skilled home care services.
- Plan for the skilled home care needs of patients in a timely manner to ensure expeditious movement to the appropriate level of care.
- Implement a comprehensive home care plan based on the in-depth assessment of the patient's needs, including coordination of activities with family, informal supports, and other community resources.
- Maintain working knowledge of third-party reimbursement guidelines and regulation standards.
- Function as a home care service resource person.
- Perform job functions in a fiscally responsible manner consistent with the mission and vision.
- Assist with orienting new nursing personnel to department policies, procedures, and practices.
- Graduate from an accredited School of Nursing.
- Bachelor's Degree in Nursing, preferred.
- Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date.
- Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
The salary range for this position is $75,020-$126,250/year. Northwell Health is an equal employment opportunity employer and welcomes applications from diverse candidates.
