Compensation: $16.00-$17.25 per hour
The Community Services Provider plays a crucial role in delivering direct support through the execution of Habilitation Plans. Key responsibilities include:
- Ensuring the safety and comfort of individuals receiving services through direct supervision and care.
- Accompanying individuals on community outings, appointments, or recreational activities to promote community engagement.
- Planning and developing inclusive activities that foster community involvement.
- Building and maintaining positive partnerships with local businesses.
- Assisting individuals with daily living tasks and personal care routines.
- Encouraging and supporting the development of skills for independent living.
- Collaborating in the creation of Habilitation Plans and adhering to these plans for effective training and support.
- Providing guidance and assistance to help individuals achieve their personal goals and enhance their living skills.
- Supervising individuals during program hours and community activities to ensure their health and safety.
- Completing necessary documentation in accordance with established regulations, policies, and billing practices.
Essential Qualifications:
- Strong interpersonal, written, and verbal communication skills, along with effective time management and organizational abilities.
- Proficiency in computer usage, including knowledge of agency operating systems and relevant software.
- Access to the internet on a regular basis.
- Ability to demonstrate sound judgment in both routine and emergency situations.
- Physical capability to perform required certifications and lift up to 50 pounds.
Preferred Qualifications:
- High School Diploma or GED, with experience or knowledge in working with individuals with developmental disabilities.
- Possession of a valid driver's license that meets the agency's insurance requirements.
