Nurse Case Manager

About the RoleThis is a full-time position working Monday-Friday, 7:30a-4:00p/8:00a-4:30p. As a Registered Nurse - Case Management, you will be accountable for the execution, adaptation, and outcome of care management as prescribed in the initial Utilization Review Plan. You will work closely with patients, staff, physicians, and other departments to facilitate multidisciplinary decision-making and ensure high-quality patient care.
Responsibilities- Assist with developing departmental goals, standards, and objectives that support the strategic plan and vision of the organization.
- Assess the quality of patient care delivered and coordinate patient care services with patients, staff, physicians, and other departments.
- Create and foster an environment that encourages professional growth and integrates evidence-based practices into operations and clinical protocols.
- Work with physicians, nurses, ancillary staff, and social services at the point of care to facilitate multidisciplinary decision-making that is consistent with the goals and objectives of the plan of care and the wishes of both the patient and family.
- Coordinate care and services within the case-managed population and educate physicians and staff in Case Management standards and assist them in meeting the regulations and standards as requested by Joint Commission, OSHA, CMS, HCFA, AHCA, and other regulatory agencies.
Applicants should have a current state RN license and possess a bachelor's degree from an accredited nursing school. Additional requirements include Basic Life Support certification within 30 days of hire, ACLS and PALS within six months of hire, and one year of experience preferred. Critical thinking skills, decisive judgment, and the ability to work with minimal supervision are essential for this role.
