Overview:
Adoration Home Health and Hospice is seeking a dedicated Physical Therapist to join our team in Shelbyville. As a home health therapist, you will make intermittent home visits to our skilled nursing patients in the area. This is a unique opportunity to bring flexibility and compassion to your career in home health.

Responsibilities:

  • Conduct thorough physical therapy evaluations and develop personalized treatment plans for patients based on physician orders.
  • Establish measurable goals and collaborate with the treatment team to ensure patients achieve maximum independence and function.
  • Maintain open communication with patients, families, and healthcare professionals to ensure seamless care.
  • Document services provided accurately and timely to ensure proper billing.
  • Supervise Physical Therapist Assistants and Aides in direct patient care and related activities.

Qualifications:

  • Minimum Bachelor's Degree in Physical Therapy from an accredited program.
  • Current, unrestricted license as a Physical Therapist in the state of operation.
  • Current BLS or CPR certification in accordance with state regulations.
  • Minimum one year of PT experience preferred.
