Responsibilities- Develop and implement individualized care plans to meet the unique needs of patients, ensuring timely and effective communication with the healthcare team.
- Collaborate with physicians and other healthcare professionals to coordinate care and ensure seamless transitions between settings.
- Provide education and support to patients and families, promoting health literacy and self-management skills.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Stay current with best practices and advances in family medicine, applying knowledge to improve patient outcomes and enhance the quality of care.
- Master's degree in Nursing from an accredited program.
- Current registration as a Professional Nurse in New York and registration as a Nurse Practitioner in New York.
- Certification as a Nurse Practitioner in a clinical area of practice, such as family medicine.
- Experience in clinical nursing practice, preferably in family medicine, with a focus on patient-centered care and collaboration with interdisciplinary teams.
- At least five years of experience in clinical nursing practice, with a focus on family medicine and patient-centered care.
- Previous experience as a nurse practitioner, with a strong track record of delivering high-quality care and contributing to quality improvement initiatives.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
