We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Home Health Clinical Coordinator. As a key member of our interdisciplinary team, you will play a vital role in ensuring the delivery of high-quality, patient-centered care to our PACE participants.
Responsibilities- Coordinate the day-to-day operations of in-home services for our PACE participants, including coordination with contracted home health agencies and the PACE program.
- Act as a liaison between the home health agency and the PACE program, ensuring seamless communication and coordination of care.
- Utilize nursing skills to assess participants in their home environment and coordinate plans of care with appropriate resources.
- Provide health education and treatments as appropriate, and participate in interdisciplinary team meetings to ensure compliance with competency requirements and thorough documentation.
- Plan and implement health and wellness initiatives in the home setting, utilizing telehealth, assistive technology, and community resources.
- Current licensure as a Registered Nurse with the North Carolina Board of Nursing.
- Minimum of 2 years of experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting.
- Minimum of 1 year of experience working with a frail or elderly population.
- Willingness to work in home environments of patients who may have values or standards different from your own.
