We are seeking a dedicated LPN (Licensed Practical Nurse) to join our team as a Healthcare Clinician for Home Care and Patient Support. This role will involve providing exceptional care to patients in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement personalized care plans.
The estimated salary for this position is around $65,000 - 85,000 per year, depending on experience and qualifications. Our comprehensive total rewards package includes competitive pay, generous paid time off, great health and wellness benefits, retirement savings, education assistance, and more.
About the Role:
This LPN position requires a strong commitment to patient-centered care, effective communication skills, and the ability to work collaboratively with our multidisciplinary team. As a Healthcare Clinician, you will:
- Support care plans for each patient, helping them achieve progress toward their goals for improved health and independence.
- Develop and document individualized plans of care with specific care interventions and expected measurable patient outcomes.
- Evaluate progress toward goals and revise plans of care as needed.
- Establish, document, and implement personalized teaching plans for patients and their caregivers to help them achieve independence at home.
- Perform nursing procedures consistent with homecare protocols.
- Initiate and participate in regularly scheduled case conferences, which include all disciplines.
- Maintain necessary clinical records, collect data, and prepare reports on activities.
- Communicate regularly with the patient's physician, home healthcare team, and other key members of the care team.
