Responsibilities- Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Assist with medication management, rehabilitation services, and group activities.
- Engage patients in one-on-one exercises and promote a positive and supportive environment.
To be successful in this role, you will need:
- A current LPN license and AHA BLS CPR certification.
- Basic computer skills and experience with Synquest, IV Pump, Phillips Monitoring System, Accucheck, and EKG Machine.
- Ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare teams.
