Position: Registered Nurse Case Manager, Hospice - The RN Case Manager is responsible for planning, organizing, directing, and delivering hospice care tailored to the specific needs of patients and their families. This role involves conducting thorough assessments of both psychosocial and physical conditions, and collaboratively developing a comprehensive care plan with the Interdisciplinary team.
Work Schedule: Full-time, Day shifts from Monday to Friday, including an on-call rotation.
Compensation: Annual Salary Range: $81,960 - $94,440, based on experience, plus additional compensation for on-call duties.

Service Area: The RN Case Manager will provide in-home care across a designated territory, primarily in the southern regions of the Twin Cities Metro area.

Qualifications:
- Graduate from an accredited nursing program.
- Current RN license in good standing in the state of Minnesota.
- At least twelve (12) months of nursing experience within the last five (5) years, or current clinical knowledge as assessed by the DHCS if lacking recent experience.
- If a recent graduate, 1200 verified hours of nursing experience within the last twelve (12) months or completion of a company-approved Preceptor Program.
- Preferred: One (1) year of experience in hospice care.
- Valid driver's license.
- Proof of valid auto liability insurance.
