About the Role:As a LPN/LVN, you will be responsible for providing direct patient care, including administering medications, performing treatments, and monitoring patient progress. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
You will have the opportunity to work with a diverse range of patients, including those with stroke, spinal cord injuries, neurological disorders, joint replacements, multiple trauma, and cardiac/pulmonary conditions. Our rehabilitation hospital is equipped with state-of-the-art technology and a team of experienced healthcare professionals who are dedicated to providing exceptional care.
Responsibilities:- Provide direct patient care, including administering medications, performing treatments, and monitoring patient progress.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Work closely with our interdisciplinary team to ensure seamless communication and coordination of care.
- Participate in patient education and counseling to promote independence and self-care.
- Contribute to the development and implementation of quality improvement initiatives to enhance patient outcomes.
- Current LPN/LVN licensure appropriate to state regulations.
- Current CPR certification.
- One or more years of experience within an inpatient medical-surgical or general hospital setting preferred.
- Rehabilitation experience preferred.
