Job SummaryWe are seeking a skilled and dedicated Home Health Physical Therapist to join our dynamic team. As a Home Health Physical Therapist, you will play a crucial role in providing specialized rehabilitation services to our patients within the familiar and supportive environment of their homes.
Key Responsibilities- Conduct Comprehensive Assessments
Conduct thorough assessments of patients' physical conditions to determine rehabilitation needs, taking into account their medical history, current health status, and functional abilities.
Develop Personalized Treatment PlansDevelop and implement individualized treatment plans in collaboration with the healthcare team, including goals, objectives, and interventions tailored to each patient's unique needs and circumstances.
Provide Hands-On Physical Therapy InterventionsProvide hands-on physical therapy interventions to improve patients' strength, flexibility, balance, and coordination, using evidence-based practices and techniques.
Educate Patients and FamiliesEducate patients and their families on therapeutic exercises, mobility aids, and strategies to promote independent living, empowering them to take an active role in their rehabilitation and recovery.
Monitor and Document ProgressMonitor and document patients' progress, adjusting treatment plans as necessary to ensure optimal outcomes and patient satisfaction.
Collaborate with Healthcare ProfessionalsCollaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care and coordination of services.
Maintain Accurate RecordsMaintain accurate and up-to-date patient records, adhering to regulatory requirements and company policies.
Communicate EffectivelyCommunicate effectively with patients, families, and caregivers to ensure a positive and supportive therapeutic environment, addressing concerns and needs in a timely and professional manner.
Qualifications- Education and Licensure
Bachelor's or Master's degree in Physical Therapy from an accredited institution, and current state licensure as a Physical Therapist.
Experience and SkillsPrior experience in home health is preferred, but not required. Strong clinical and critical thinking skills, excellent communication and interpersonal abilities, and the ability to work independently and as part of a collaborative healthcare team.
