Job Overview

We are seeking a highly motivated Child and Family Case Manager to join our team. The ideal candidate will have a deep understanding of human services, resources, and tools, as well as excellent communication and interpersonal skills.

Key Responsibilities
  • Develop and implement individualized plans to achieve desired outcomes;
  • Build and maintain relationships with clients, families, and community resources to facilitate access to necessary services;
  • Participate in treatment teams to develop and implement effective plans that address client needs;
  • Collect and analyze data to inform decision-making and ensure effective service delivery;
Requirements
  • A thorough understanding of human services, resources, and tools;
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations;
  • A Bachelor's degree in a Human Services-related field or equivalent experience;
  • Ability to work independently and as part of a team;
