Home Care LPN Opportunity

At Interim HealthCare of the Twin Cities, we are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality home care services to patients in the Southwest Twin Cities Metro area.

About the Role

This is a full-time position with a regular schedule of 8:00am - 5:00pm, Monday through Friday. As an LPN, you will be responsible for visiting patients in their homes, providing skilled nursing care, and collaborating with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Participate in ongoing interdisciplinary assessment of the patient
  • Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s)
  • Provide services that are ordered by the physician as indicated in the plan of care
  • Recognize and report changes in the patient's condition to the RN
  • Counsel and educate the patient and family in meeting healthcare related needs
  • Complete timely and accurate clinical notes
Requirements
  • Current license as an LPN in the State of MN
  • 2 years' experience as an LPN
  • Able to travel within geographic area serviced by the office
  • Valid driver's license, reliable transportation, and proof of auto insurance
  • CPR Certification
