Job Description

As a Personal Care Specialist with Professional Case Management, you will provide essential personal care services to clients in the comfort of their own homes. Under the guidance of our experienced RN/Case Managers, you will be assigned to specific clients and perform services as needed to maintain their personal comfort and well-being.

Key Responsibilities:

  • Provide personal care services, including assistance with activities of daily living, such as bathing, dressing, and grooming
  • Assist with light housekeeping and meal preparation
  • Monitor and record vital signs, including temperature, blood pressure, and pulse
  • Perform skilled transfers and range of motion exercises
  • Document all services provided in a timely and accurate manner

Requirements:

  • Active certification as a CNA in the state(s) of practice
  • Current CPR certification and TB test
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
