LHC Group is seeking a dedicated Care Transition Coordinator to enhance our service delivery. The Care Transition Coordinator plays a pivotal role in implementing our sales strategy aimed at expanding our market presence by developing relationships within the medical community and promoting our comprehensive services while adhering to budgetary constraints. This position is crucial for ensuring a smooth transition for patients moving from a healthcare facility to our agency for their post-acute care needs.
Key Responsibilities
- Achieve monthly production targets and manage admit budgets for designated locations while ensuring responsible use of financial resources.
- Develop and execute strategic plans on a weekly, monthly, and quarterly basis to enhance market share within assigned facilities.
- Assess patient suitability for home care following established guidelines.
- Conduct face-to-face meetings with patients to educate them about our agency and coordinate care with their primary physician.
- Provide the Executive Director with insights into patient needs for branch approval and ensure proper documentation in our systems.
- Facilitate the organization of transfer orders and coordinate additional services required by the patient.
- Ensure all patient needs identified by the referral source are addressed and documented.
- Collaborate closely with the Executive Director and Clinical Director to foster a growth-oriented vision focused on the needs of patients and referral sources.
- Handle all sales administration tasks, including compliance with expense entries and payroll documentation.
- Educate patients on the importance of follow-up appointments and medication management prior to discharge.
- Act as a liaison between our agency and healthcare providers for newly referred and existing patients.
- Communicate with discharge planning teams regarding active patients transitioning from home health to facility care.
- Provide feedback to the case management team about patient readmissions and non-admit decisions.
- Maintain patient confidentiality at all times.
- Understand and articulate the benefits of our services, effectively communicating with the medical community through sales calls and presentations.
- Perform any additional tasks as assigned.
Experience Requirements
- Minimum of one year of experience in home health or hospital case management.
- Preferred one to three years of experience in medical marketing.
- Current RN, LPN, SW, or PT licensure in the state of practice.
- Certification from a technical school demonstrating strong clinical knowledge.
- Valid driver's license and reliable transportation.
- Exceptional verbal and written communication skills.
- Strong organizational abilities and capability to manage competing priorities.
- Thorough understanding of home health eligibility criteria and coverage guidelines.
- Proficient in computer skills, including Microsoft Outlook.
- Excellent presentation, negotiation, and relationship-building skills.
- Ability to work independently with minimal supervision and be self-motivated.
