FT Registered Nurse Opportunity in Home Health

Comfort Home Care is seeking an experienced Registered Nurse to join our team as a Home Health RN. As a FT RN, you will have the opportunity to make a real difference in the lives of our patients while enjoying a flexible schedule and competitive pay.

About the Role

As a Home Health RN, you will be responsible for providing high-quality patient care in the comfort of their own homes. You will conduct initial and ongoing assessments, develop and implement individualized care plans, and coordinate services with other healthcare providers.

Our ideal candidate will have a strong background in home health care, excellent communication skills, and a passion for delivering compassionate and personalized care.

Key Responsibilities
  • Conduct initial and ongoing assessments to identify patient needs and develop individualized care plans
  • Provide high-quality patient care in the comfort of their own homes
  • Coordinate services with other healthcare providers to ensure seamless care
  • Develop and implement individualized care plans to meet patient needs
  • Communicate effectively with patients, families, and healthcare providers to ensure optimal care

Requirements
