About the Role

As an LVN, you will be responsible for assessing the nursing needs of our residents, developing and implementing individualized care plans, and collaborating with our interdisciplinary team to ensure seamless care delivery. You will also have the opportunity to work closely with our care partners to ensure that each resident receives the highest level of care and attention.

Responsibilities
  • Assess and document resident nursing needs
  • Develop and implement individualized care plans
  • Collaborate with the interdisciplinary team to ensure seamless care delivery
  • Provide direct care to residents, including administering medications and treatments
  • Monitor and report any changes in resident condition to the charge nurse or physician
Requirements
  • Current LVN licensure in the state of California
  • Current CPR certification
  • Prior nursing experience in a skilled nursing or post-acute care setting
