As a Home Health Physician with South Region OPS Med Groups, you will be an integral part of our team working to change the face of health care for seniors. Our model provides a comprehensive, coordinated model of care to a predominantly Medicare-aged population. You will manage care in the patient's home, with occasional visits to the office or other facilities as needed.

Key highlights of this position include:

      • Collaborate with a multi-disciplinary team of health care providers that includes physicians, NP's, PA's, RN case managers, social workers, and clinic staff.
      • Our model prioritizes quality/value-based outcomes.
      • Facilitate patient and family decision making regarding treatment decisions and end of life care and serious illness discussions.
      • Treat acute and chronic conditions, including diabetes, hypertension, kidney disease, and other chronic medical conditions.
      • Prescribe pharmacological and non-pharmacological interventions.

Required Qualifications:
      • Doctor of Medicine (M.D.), Doctor of Osteopathy (D.O.), or M.B.B.S.

      • Bilingual (English/Spanish) fluency.
      • Valid DEA license.
      • Valid Driver's license and reliable transportation.
