Cataldo Ambulance Business Trust is seeking a highly skilled and dedicated SmartCare Paramedic to join our team. As a SmartCare Paramedic, you will play a critical role in delivering high-quality patient care in the comfort of patients' homes.
Job SummaryThe SmartCare Paramedic is responsible for providing comprehensive in-home care to patients, including assessments, treatments, and monitoring. You will work closely with patients, families, and healthcare teams to ensure seamless care coordination and optimal patient outcomes.
Responsibilities- Deliver high-quality patient care in accordance with clinical policies, statewide protocols, and standards of care.
- Attend to patients in a courteous and professional manner, ensuring their comfort and dignity.
- Monitor remote patient tracking and respond promptly to any changes or concerns.
- Participate in discharge planning and coordinate care transitions to ensure smooth patient handovers.
- Conduct initial in-home assessments and ongoing home visits to monitor patient progress and adjust care plans as needed.
- Communicate effectively with EMS agencies, physicians, and other healthcare providers to ensure seamless care coordination.
- Fully state certified as an EMT-P.
- Certifications in Advanced and Basic Life Support.
- Valid driver's license and an acceptable driving record through a DMV background check.
- 2 years minimum experience as a Paramedic.
- Be in good clinical standing.
- Possess strong customer service and communication skills.
- Be able to work independently and as part of a care planning team.
- Pass a background check, including CORI.
- Pass a drug screen and lift test.
- Commitment to providing exemplary customer service.
- Advanced Medical Life Support (AMLS) and either Pediatric Advanced Life Support (PALS) or Emergency Pediatric Care (EPC) are preferred.
- The successful completion of the Community Health Paramedicine Training Program within 6 months of hire or at the first course offering.
- Starting wages $40/hour, commensurate with experience.
