We are seeking a skilled Occupational Therapist to join our team in providing high-quality rehabilitation services to patients in their homes. As a Home Health Occupational Therapist, you will play a vital role in helping patients maintain independence and improve their daily living skills.

Job Responsibilities
  • Assess patients' needs and develop tailored therapeutic programs to meet their goals.
  • Collaborate with healthcare teams to ensure effective care and coordination of services.
  • Provide expert rehabilitation services in familiar surroundings, promoting patient comfort and trust.
  • Document patients' care observations, interventions, and evaluation on the day services are rendered.
Requirements
  • Degree from an accredited Occupational Therapy Program.
  • A minimum of six months of occupational therapy experience preferred.
  • Home Health experience a plus.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation.
