We are seeking a highly skilled Speech Therapist to join our team at Trinity Health. As a Speech Therapist, you will play a critical role in providing professional speech and audiology diagnostic and therapeutic services to our clients.
Key Responsibilities- Client Assessment & Planning: Conduct thorough assessments of clients to identify their needs and develop personalized treatment plans.
- Visit Prioritization: Prioritize and plan visits to ensure effective and efficient service delivery.
- Regulatory Compliance: Maintain knowledge of relevant laws and regulations, including the Organizational Integrity Program and agency policies.
- Care Coordination: Make referrals to other disciplines and services as needed, and coordinate care to ensure effective service delivery.
- Intervention & Education: Implement preventive, therapeutic, and rehabilitative procedures according to treatment plans, and instruct clients and their families on methods to improve communication and swallowing deficits.
- Certificate of Clinical Competence in Speech Pathology (CCC-SLP) from the American Speech and Hearing Association, or working toward certification with required supervised experience.
- Licensed to practice as a Speech Pathologist in Connecticut.
- Ability to function with minimal direct supervision, maintaining a professional relationship with clients and their families.
- Valid Driver's License and reliable transportation.
- $5,000 Sign-On Bonus
