About the Role

We are seeking a skilled and compassionate Physical Therapist to join our team at Revival Home Health & Hospice. As a Physical Therapist, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Conduct thorough physical therapy evaluations and develop personalized treatment plans to meet the unique needs of each patient.
  • Provide one-on-one care and support to patients in their homes, promoting independence and optimal function.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Document patient progress and maintain accurate records in accordance with company policies and regulatory requirements.
  • Supervise and mentor Physical Therapist Assistants and Physical Therapy Aides as needed.
Requirements
  • Graduate of a Physical Therapist Education program approved by CAPTE.
  • Current Physical Therapist licensure in the state and American Heart Association CPR certification.
  • At least two years of experience in adult physical therapy, with home health experience preferred.
  • Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills.
