Medical Scribe Specialist

Oak Street Health is seeking a highly skilled Medical Scribe Specialist to support our primary care providers with clinical documentation. As a key member of our care team, you will play a vital role in ensuring accurate and timely documentation of patient encounters.
About the Role
This full-time position is based in Arizona and offers a hybrid work arrangement, allowing for 2 days of remote work and 3 days of on-site work at one of our clinics. You will be responsible for documenting patient encounters, including histories, assessments, plans, and physical exams, as well as assigning CPT and ICD-10 codes. In addition, you will provide administrative support to your provider and care team, placing orders, addressing tasks, and supporting clinical documentation improvement initiatives.
Responsibilities
• Document patient encounters (80%): Join the provider in the exam room to observe patient visits, document patient encounters in a structured note, assign CPT and ICD-10 codes, prepare After Visit Summaries, and consult with the provider to ensure accurate and specific documentation.
• Clinical documentation improvement (10%): Request and review medical records, leverage Oak Street's population health tools to support clinical documentation improvement, prepare for and support Daily Huddles and Clinical Documentation Reviews, and consult with the provider on clinical documentation opportunities.
• Administrative support (10%): Place orders, address tasks, support the care team with additional responsibilities related to clinical documentation, and perform other duties as assigned.
Requirements
We are looking for an individual with prior clinical experience, knowledge of medical terminology, and advanced listening and communication skills. Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve is required where indicated. The ideal candidate should commit to at least 1 year in this role and have excellent job attendance, ability to work in-person in our clinics, and adapt to new workflows and learn new concepts and skills quickly.
