Job Description:
As a Certified Occupational Therapy Assistant (COTA) at HexaQuEST Health, Inc., you will play a vital role in providing high-quality care to our patients. We are seeking a skilled and dedicated COTA to join our team in Portland.

Responsibilities:
• Ensure that all necessary paperwork, including prescreen forms and attachments, is completed and submitted with the candidate.
• Verify that resumes and license verification are attached to the submission.
• Conduct thorough background checks, including county, federal, and multi-state searches, as well as a nationwide sex offender search and HHS/OIG search.
• Obtain a 2-step PPD or Quantiferon test result and proof of positive PPD and chest X-ray, if required.
• Review the credentialing checklist prior to submission.

Requirements:
• Currently active state license.
• Less than 1 year of experience.
• Minimum guaranteed hours: 32.
• EMR system: No Listing.

Work Schedule:
• Mon-Fri shifts.
