Job Summary

We are seeking a Physician Assistant to join our team as a Locum Tenens State Traveler in Vermont. As a Physician Assistant with Matrix Medical Network, you will have the opportunity to work with a diverse range of patients and healthcare professionals, and to make a meaningful difference in the lives of those you serve.

Responsibilities
  • Conduct home risk assessments for patients in Vermont
  • Review and observe patients' current health, medical history, medication adherence, social environment, and other risks
  • Develop and implement care plans to address identified risks and improve patient outcomes
  • Collaborate with healthcare professionals and patients to ensure seamless care transitions
  • Document patient information and care plans in accordance with Matrix Medical Network's guidelines and protocols
Requirements
  • Current Nurse Practitioner licensure in Vermont
  • AANP, ANCC, or AACN certification
  • Current BLS, ACLS, or CPR certification
  • Strong computer skills and familiarity with electronic health record software
  • Excellent verbal and written communication skills
  • Ability to travel and work in a variety of settings, including patients' homes and healthcare facilities
