Key Responsibilities:- Provide comprehensive primary care services to patients, including diagnosis, treatment, and education.
- Collaborate with the core PACT Team and expanded PACT Team to develop and implement patient care plans.
- Order and perform diagnostic tests, interpret results, and provide patient notification and follow-up care.
- Counsel patients on medication use, educate on health maintenance, and assess mental health issues.
- Complete clinical reminders, documentation, and compliance with VA guidelines.
- Graduate of an accredited school of nursing and MSN degree.
- Certification by applicable professional organization.
- Minimum one-year clinical experience as a CRNP in a related primary care or ambulatory care setting.
- Credentialed and in good standing through the Veterans Health Administration (VA).
- Valid, unrestricted license in the state of desired employment.
- Current certification in Basic Life Support and ACLS.
- Unrestricted DEA registration.
- Proficiency in written and spoken English.
