Key Responsibilities:
  • Provide comprehensive primary care services to patients, including diagnosis, treatment, and education.
  • Collaborate with the core PACT Team and expanded PACT Team to develop and implement patient care plans.
  • Order and perform diagnostic tests, interpret results, and provide patient notification and follow-up care.
  • Counsel patients on medication use, educate on health maintenance, and assess mental health issues.
  • Complete clinical reminders, documentation, and compliance with VA guidelines.
Requirements:
  • Graduate of an accredited school of nursing and MSN degree.
  • Certification by applicable professional organization.
  • Minimum one-year clinical experience as a CRNP in a related primary care or ambulatory care setting.
  • Credentialed and in good standing through the Veterans Health Administration (VA).
  • Valid, unrestricted license in the state of desired employment.
  • Current certification in Basic Life Support and ACLS.
  • Unrestricted DEA registration.
  • Proficiency in written and spoken English.
