Nurse Care Specialist

About the Role:
We are seeking a skilled and compassionate Registered Nurse to join our team at LHC Group. As a Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence and improving health outcomes.
Key Responsibilities:
- Conduct comprehensive assessments of patients' physical and emotional needs
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
- Provide education and support to patients and families on disease management, medication, and self-care
- Collaborate with healthcare professionals to ensure seamless care transitions and coordination
- Document patient progress and outcomes in a timely and accurate manner
Requirements:
- Current RN licensure in the state of practice
- Current CPR certification
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
