Position SummaryThe Physical Therapist is tasked with designing and executing tailored treatment programs for individual clients, adhering to the principles and practices of physical therapy and in alignment with physician directives.
Key Responsibilities- Conduct thorough evaluations of clients, assessing range of motion, flexibility, muscle strength, and other physical capabilities.
- Develop and implement personalized treatment plans to address identified issues, setting realistic goals with clients and adjusting plans as necessary.
- Utilize various physical therapy techniques to enhance client function and performance, including therapeutic exercises and mobility training.
- Provide guidance and instruction on treatment procedures to clients, families, and other healthcare team members.
- Assess the need for adaptive equipment and instruct clients and families on its use, ensuring safety in the home environment.
- Document client progress and treatment outcomes accurately and in a timely manner, adhering to agency policies.
- Collaborate with other healthcare professionals to ensure comprehensive care for clients.
- Participate in staff training and educational programs as required.
To be successful in this role, candidates must possess:
- A Bachelor's degree in Physical Therapy from an accredited institution.
- A minimum of two years of experience in physical therapy, with a preference for home health experience.
- An active Colorado Physical Therapist license and current CPR certification.
- Strong communication skills and the ability to work effectively with clients and team members.
The work environment is dynamic, as care is provided in clients' homes, which may present varying conditions. Employees may encounter outdoor weather, household pets, and other environmental factors.
Physical DemandsThis position requires the ability to perform physical tasks, including lifting and moving clients as necessary, and may involve travel for home visits.
