RN Case Manager

As a skilled RN Case Manager at Della Infotech, you will play a vital role in optimizing patient outcomes by collaborating with multidisciplinary teams. Your expertise will ensure seamless care coordination, from pre-admission to post-discharge, for a defined population of patients. You will serve as a liaison between patients, families, nurses, physicians, and healthcare benefactors to ensure efficient hospitalization processes.
Key Responsibilities:
- Lead the coordination of patient care with other disciplines, monitoring the appropriateness and timeliness of care.
- Ensure the interdisciplinary care plan aligns with the patient's clinical course, continuing care needs, and covered services.
- Discuss with physicians the appropriateness of resource utilization, consultations, treatment plans, estimated length of stay, and discharge plans.
- Coordinate patient transfers to tertiary centers, including the transfer of patient information required for ongoing treatment and services.
- Provide oversight and collect data for regulatory and accreditation compliance.
- Bachelor's degree in nursing (BSN): Preferred
- Experience in a care management role: Preferred
- Registered Nurse (RN) licensure in the state of practice: Required
- Case management certification
