MDS Coordinator Responsibilities:

As the MDS Coordinator, you will serve as the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) Coordinator for our center. Your responsibilities will include reviewing and processing MDS assessments in accordance with CMS and LTCI regulations and guidelines.


  • Develop and revise resident Interdisciplinary Care Plans based on RAI/MDS information
  • Collaborate with nursing caregivers and other disciplines to ensure accurate care plan goals
  • Communicate effectively with various disciplines to facilitate timely completion and transmission of RAI/MDS
  • Prepare accurate reports on Quality Measure/Quality Indicators (QM/QI) from RAI/MDS data
  • Participate in Quality Improvement initiatives
  • Implement educational plans to address changing program or service needs
