Job Summary

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Full Time Registered Nurse Case Manager in Home Healthcare. As a RN Case Manager, you will be responsible for managing the cases of our home health patients, providing high-quality care and support to ensure the best possible outcomes.

Responsibilities
  • Manage a caseload of home health patients, providing comprehensive care and support to ensure the best possible outcomes.
  • Conduct thorough assessments of patients' needs and develop individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families to promote health and wellness.
  • Document patient information and progress in a timely and accurate manner.
Requirements
  • Current RN licensure in the state.
  • At least one year of recent experience in subacute or wound care patients, such as Rehab, Hospital, Home Health, or Hospice experience.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Proficiency in electronic documentation and other relevant software systems.
