PennState Health is seeking a highly skilled Clinical Pharmacist to join our team in Hummelstown, Pennsylvania.
Key Responsibilities- Develop and implement comprehensive drug therapy plans for patients in specific pharmacotherapy specialty areas.
- Collaborate with healthcare teams to provide education and training on medication management and safety.
- Conduct multidisciplinary rounding and patient monitoring to ensure optimal patient outcomes.
- Design and deliver education programs for pharmacists, pharmacy residents, and students on medication therapy and patient care.
- Provide in-service education to physicians, nurses, and medical students on medication management and safety.
- Doctor of Pharmacy degree required.
- ASHP-accredited PGY1 and relevant PGY2 specialty residency or equivalent experience required.
- Licensed or eligible for licensure to practice pharmacy in the state of Pennsylvania.
