Key Responsibilities
- Deliver personal care as assigned, responding promptly to patient requests.
- Notify the Case Manager immediately if unable to fulfill a specific task.
- Utilize effective communication techniques with patients and families, ensuring confidentiality and respect.
- Ensure patient safety by using equipment correctly (e.g., foot stools, side rails, O2).
- Provide personal care services, including bathing, oral hygiene, and changing bed linens as required.
- Assist patients with dressing and undressing as necessary.
- Plan and prepare nutritious meals, including shopping when assigned.
- Support patients during meals as needed.
- Measure and record vital signs, including temperature, pulse, respiration, and blood pressure, as per care plans.
- Monitor and care for patients' skin to prevent breakdown.
- Report any significant changes in the patient's condition to the Case Manager.
- Be aware of the dynamics involving caregivers and other individuals in the patient's environment.
- Assist with ambulation and exercises as directed by nursing staff.
- Provide normal range of motion exercises and assist with positioning as instructed.
- Help with self-administered medications, as permitted by state regulations.
- Assist with bedpans and urinals as needed.
- Support light laundry tasks as assigned.
- Perform range of motion exercises and other simple procedures as directed.
- Offer respite care for patients and their families when on-site.
- Maintain cleanliness and orderliness in the patient's living area as assigned.
- Adhere to the organization's documentation standards and professional conduct guidelines.
- Participate in quality improvement initiatives and activities.
- Respect the personal needs and environment of patients and their families.
Qualifications
- Completion of state-required training.
- A minimum of seventy-five hours of classroom and supervised practical training, with sixteen hours of classroom instruction completed prior to practical training.
- At least six months of experience in providing personal care.
- Must be at least eighteen years old.
- Ability to read, follow written instructions, and document care provided.
- Understanding of hospice philosophy and comfort in providing care to terminally ill patients.
- Positive references from previous employers.
- Ability to work independently with minimal supervision.
- Comfortable with issues surrounding death and dying.
- Maintain a calm demeanor in patients' homes.
- Current CPR certification is required.
- Exhibit patience, tact, and good personal hygiene.
- Successfully complete a probationary period to demonstrate competency in the required qualifications.
- Strong communication skills, both oral and written.
- Good organizational abilities.
