Job Summary

We are seeking a skilled and compassionate Registered Nurse Case Manager to join our team. The RN Case Manager will work collaboratively with our interdisciplinary team to provide patient-centered care and support to individuals and families in our community.

Key Responsibilities
  • Conduct patient and family assessments to identify physical, emotional, and spiritual needs
  • Develop and implement individualized plans of care to meet patient and family needs
  • Provide education and support to patients and families on pain management, end-of-life care, and other relevant topics
  • Collaborate with the interdisciplinary team to ensure seamless care coordination
  • Document patient and family interactions, assessments, and care plans in our electronic health record
Requirements
  • ADN or BSN required
  • Current, unrestricted Registered Nursing License in state of employment
  • One-year nursing experience, preferred
  • Valid driver's license, auto insurance, and access to a reliable vehicle
  • Ability to develop, maintain, and apply knowledge of hospice and end-of-life process
  • Ability to maintain professional boundaries with patients, families, and co-workers
  • Ability to create and follow plans of care, and patient, family, and clinical staff directions
