Job Description

The Oncology Nurse Navigator is responsible for coordinating the interdisciplinary team of providers to best serve the newly diagnosed cancer patient. This process includes assessment, education, and continuity of care for the patient and family.

In collaboration with the Medical Director of Cancer Services, medical oncologists, radiation oncologists, and appropriate specialists, coordinates patient services and outreach components of the program. This includes monitoring patients through screening programs and following patients diagnosed through their cancer treatment and up until survivorship.

The successful candidate will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. A graduate degree in nursing or related field, five to seven years of nursing experience, and current Connecticut RN license are required.
