LPN - Licensed Practical Nurse Supervisor - Role Summary:

  • As vital members of our interdisciplinary team, Licensed Practical Nurses (LPNs) are responsible for overseeing the daily clinical care of each resident and sub-acute patient. Our LPNs have a unique opportunity to gain extensive clinical experience while delivering advanced care in a nurturing environment.
  • Direct nursing responsibilities include planning, developing, organizing, implementing, and assessing the care of residents to ensure the highest standards of quality care and compliance with regulations.
  • A valid state LPN license is mandatory.

Key Responsibilities (LPN - Licensed Practical Nurse):

  • Administer medications and treatments to assigned residents as prescribed by physicians, adhering to the five rights of medication administration.
  • Supervise and guide the personal care tasks and nursing procedures performed by Nursing Assistants on the assigned team.
  • Assist Nursing Assistants with direct resident care as needed.
  • Report any unusual symptoms, changes in condition, daily needs, and progress of residents to the Unit Manager/Assistant Unit Manager or Charge Nurse Supervisor.
  • Follow the individualized Plan of Care for each resident and monitor the performance of Nursing Assistants in executing the care plan.
  • Receive controlled substances from the supervisory nurse and manage all aspects of their storage and distribution, ensuring proper and safe handling of medications.
  • Document as required in care plans, Nursing Notes, Medical Administration Records, Treatment Records, and other necessary documentation.

Qualifications (LPN - Licensed Practical Nurse):

  • Current credentials as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a valid state license are required.
  • Experience in a geriatric care setting is preferred.
  • Ability to supervise and collaborate effectively with other staff members is essential.
  • Strong verbal and written communication skills are necessary.
  • A caring and respectful attitude towards residents is required.
  • Physical stamina for continuous activity is essential.
  • Ability to perform tasks to established standards of excellence is required.
  • This role involves regular interaction with residents, colleagues, visitors, and supervisors, necessitating proficiency in reading, writing, speaking, and understanding the English language at an intermediate or advanced level.
