Job Description:
We are seeking a compassionate and experienced Travel Physical Therapist to provide high-quality care to patients in their homes. As a Travel Physical Therapist with Supplemental Health Care, you will have the opportunity to work with a variety of patients and develop your skills in a dynamic and supportive environment.

Home Health Physical Therapist Contract Details:
- Competitive hourly rate*
- SHC Home Health Physical Therapists are paid by the hour, not by the visit
- Therapist will have a designated territory of coverage
- Weekly expectation is 30 units of productivity
- Home Health contracts with SHC require previous experience with OASIS/Medicare Certified Visits
- *Estimated hourly rate may include both taxable wages and tax-free reimbursements for meals, housing, and incidentals. Please speak with a recruiter for details.

Requirements:
- OASIS/Medicare Certified Visits Experience
- Electronic Charting experience
- Minimum of 1 year of recent home health physical therapy experience (private duty does not qualify)
- Most contracts require some on-call
- Current Illinois Physical Therapist License
- American Heart Association BLS
- Valid U.S. Driver's License
