About the Role

We are seeking a skilled Home Health Physical Therapist to join our team at LHC Group. As a Physical Therapist, you will play a crucial role in providing high-quality patient care and contributing to the well-being of our patients.

Job Summary

The ideal candidate will have current licensure in the state of New Jersey, current CPR certification, and access to a dependable vehicle. The successful candidate will be responsible for assessing and evaluating patient care needs, developing treatment plans, and delivering interventions aimed at improving patient outcomes.

Responsibilities:
  • Evaluate patient functional status and physical therapy needs.
  • Assist physicians in developing therapy plans of care.
  • Observe and report patient responses to treatment and changes in condition to supervising nurses and/or physicians.
  • Treat patients to relieve pain, develop, and restore function, or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating members of the healthcare team, patients, and family/caregivers regarding plans of care, functional limitations, and progress toward goals.
Requirements:
  • Current Physical Therapy licensure in the state of New Jersey.
  • Current CPR certification.
  • Access to a dependable vehicle.
