Aveanna Private Duty Nurse LPN Job Description

Aveanna's Private Duty nurses provide one-on-one care to patients in their homes, assisting with transitions from hospital to home and ensuring continuous care. Our nurses are matched with patients based on skill, age, health condition, and acuity level.

Responsibilities:
  • Implement physicians' prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in a compliant and timely manner
  • Teach and train family members on health promotion and disease management
  • Accept and write verbal orders from physicians, with co-signature from Case Manager
  • Follow up with physicians, clients, family members, staff, and Case Manager on client problems and concerns
Qualifications:
  • Active, unencumbered license (RN/LPN/LVN) in the state of practice
  • Current CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test (current within last 12 months)
  • One year prior hands-on nursing experience
  • Reliable transportation
