We are seeking a highly skilled Occupational Therapist (OT) to join our team as a Physical Therapist Assistant (PTA) in Home Health. The successful candidate will be responsible for evaluating patient needs, establishing goals, and providing interventions to assist patients in achieving independence with activities of daily living.
Key Responsibilities- Evaluate patient needs and develop individualized treatment plans
- Provide interventions to assist patients in achieving independence with activities of daily living
- Effectively interact with patients, doctors, and other healthcare members
- Assess, plan, treat, evaluate, and implement a Plan of Care (POC) for all clients seen
- Communicate and interact with patients, families, physicians, and staff, demonstrating behavior consistent with the hospital's core values
- Complete documentation of all patient care activities in a timely manner in the electronic medical record
- Ability to transfer and position patients
- 1 year Home Health PTA experience - REQUIRED
- Valid driver's license and car insurance - REQUIRED
- COVID vaccination required - DECLINATIONS WILL NOT BE APPROVED
- OASIS and start of care experience - strongly preferred
- Manager will review local candidates. If the candidate is too far from the territory, they will redirect.
