Achieving True Self is seeking a talented Caregiver Advocate to support individuals with Intellectual Disabilities/Autism in Westmoreland County, PA.

About the Role

This is a fantastic opportunity for personal growth and development as an Intellectual Disabilities/Autism (ID/A) Habilitation Aide Specialist. You will provide direct support services to clients in their homes or community settings.

Responsibilities
  • Document services provided, collect data, and assess strategies for support
  • Provide direct support in accordance with plans and ODP guidelines
  • Support clients in home and community-based settings to acquire skills for independent living
  • Assist with self-care, health management, communication, mobility, relationships, and socialization
  • Transport clients to community activities

Requirements
  • High school diploma or GED equivalent
  • Valid driver's license with good driving record and reliable vehicle
