Diabetes Nurse Navigator

UNC Health Care is seeking a highly skilled Diabetes Nurse Navigator to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional care to patients with diabetes.
Job Summary:The Diabetes Nurse Navigator will be responsible for coordinating care for high-risk patients with diabetes, providing education and guidance to patients and their families, and collaborating with healthcare providers to ensure seamless transitions and timely access to care.
Key Responsibilities:- Provide patient education and guidance on diabetes management, including care plans and resources.
- Facilitate shared decision-making with patients and their families, and coordinate appointments, tests, and procedures as needed.
- Collaborate with healthcare providers to ensure continuity of care and timely access to services.
- Develop and lead educational events and programs to promote diabetes management and self-care practices.
- Collect and analyze data on patient outcomes and submit reports to regulatory agencies as required.
To be successful in this role, you will need:
- A Bachelor's Degree in Nursing and current NC license as a Registered Nurse.
- Minimum of three years of experience in nursing, including care of diabetes patients.
- Excellent interpersonal and customer service skills, with ability to build collaborative relationships with providers, patients, and families.
- Ability to read, write, and communicate effectively in English.
- Proficient with MS Office and able to learn new software rapidly.
