Hospice Referral Coordinator Job Summary

Patriot Hospice is seeking an experienced LPN/Referral Coordinator to join our team. As a key member of our admissions department, you will be responsible for coordinating and maintaining ongoing tracking and documentation of referrals and admissions.

Key Responsibilities

  • Coordinate referrals and admissions for Patriot Hospice and Palliative Services.
  • Document patient referrals and follow referral process to determine admission criteria.
  • Communicate with patients, families, Medical Director, and Admission Team to initiate admission process.
  • Register patients accurately, including demographic and insurance information.
  • Assemble patient clinical background and referral needs information.
  • Provide clinical information to specialists and physicians as needed.
  • Review referral details with patients as needed.
  • Assist patients with healthcare system, financial, or social barriers.

Salary and Benefits

We offer a competitive salary range of $65,000 - $80,000 per annum, depending on experience, as well as opportunities for growth and development.
