Key Responsibilities

  1. Ensure the safety of residents during transportation to medical appointments.
  2. Accompany residents throughout their medical visits, remaining with them until the appointment concludes.
  3. Follow specific protocols during each visit, including:
  • Registering residents upon arrival.
  • Presenting necessary documentation to medical staff.
Before departing from the medical facility, ensure:
  • All resident documents are collected.
  • New prescriptions are obtained.
  • Doctor's orders are confirmed.
  • Follow-up appointments are scheduled, if necessary.
Upon returning to the facility, report to the relevant personnel and provide details regarding the visit.Perform additional duties as assigned.

Qualifications

  1. Ability to assist residents in transferring to and from vehicles.
  2. Possession of a valid state driver's license.
  3. Certification as a Nurse Assistant (C.N.A.) is preferred.
  4. Maintained clean driving and criminal records.
  5. Compliance with any other state-mandated certifications.
