Position Overview:
The Licensed Nursing Assistant is responsible for performing assigned nursing tasks, homemaking duties, and other responsibilities for patients of all ages in their residences or designated facilities. This role involves:

  • Documenting and reporting patient needs and responses to the designated Case Manager or Clinical Manager.
  • Alerting the appropriate personnel to any changes in patient condition or circumstances without delay.
  • Following the Home Health Aide care plan established by the primary nurse or therapist while ensuring a safe and healthy environment.
