Job Summary:

We are seeking a compassionate and skilled Registered Nurse to join our team as a Case Manager. As a Case Manager, you will be responsible for planning, organizing, and directing hospice care for patients and their families. You will work closely with the interdisciplinary team to ensure that patients receive comprehensive and coordinated care.

Key Responsibilities:
  • Conduct initial and ongoing assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
  • Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions between settings.
  • Communicate effectively with patients, families, and healthcare providers to ensure that care is patient-centered and meets their needs.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current licensure in the state of practice.
  • One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care preferred.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
