Job Summary

We are looking for a skilled CNA to provide personalized care and support to patients in the comfort of their own homes. As a member of our interdisciplinary team, you will work closely with physicians, therapists, social workers, and Home Health Aides to deliver comprehensive care and effective treatment plans.

Responsibilities
  • Provide basic life support and patient care to hospice patients, including vital sign monitoring, medication administration, and wound care.
  • Utilize electronic medical records (EMRs) and electronic health records (EHRs) for accurate medical documentation.
  • Demonstrate knowledge of medical terminology and physiology in patient interactions, ensuring clear and effective communication.
  • Ensure HIPAA compliance in all aspects of patient care, maintaining confidentiality and security of sensitive information.
  • Assist with catheter care as needed, following established protocols and guidelines.
  • Collaborate with the healthcare team to deliver compassionate end-of-life care, prioritizing patient comfort and dignity.
Requirements
  • High school diploma or equivalent required.
  • CNA certification and BLS certification required.
  • Knowledge of medical terminology and physiology essential.
  • Experience in catheter care and end-of-life care preferred.
  • Valid driver's license required.
  • Ability to review and maintain accurate medical documentation.
